Complete the full screening
Walk through a physician-designed intake covering cardiac history, medical history, orthopedic health, vitals, and general wellness. Upload a vaccination card, your camp or school's own form, and a brief 30–60 second movement video.
Receive your signed clearance
A board-certified physician reviews every submission — not an algorithm — and emails the signed PDF, on your school or camp’s own form, usually within 24 hours. A same-day rush option is available at checkout.
What to have ready
- A recent height & weight — for your child.
- A vaccination card — a CDC card, MyChart screenshot, or pediatrician printout all work.
- Your camp or school's form — if they handed you a specific one — we complete and sign that exact form.
- A phone — to record a short movement video of your child.
- A blood-pressure reading — optional, but helpful if you have a recent one.
Verifiable by your school
Every clearance carries a QR code. Your school, camp, or league scans it to confirm it's genuine, straight from our records — protection a paper note can't offer.
Cleared, or your money back
If the physician can't clear your child, we refer instead and refund the visit in full. Not licensed in your state? You're never charged. And if your school or camp has any questions about the clearance, we'll communicate with them directly to verify it and resolve them.
