What is SportSlip?

SportSlip is a telehealth service that provides physician-reviewed sports and camp physical clearances. You complete a health screening, record a short video of your child, upload the form your camp or school handed you, and a board-certified physician fills it out and signs it — all from your phone.

Is this a real doctor or AI?

Every clearance is personally reviewed and signed by Dr. Adam Z. Kawalek, MD — a board-certified internal medicine physician (NPI 1326223306). Not an algorithm, not a template, not a rotating panel.

How is this different from a quick physical at urgent care or a pharmacy clinic?

Completeness, mostly. Rushed in-person physicals routinely compress or skip parts of the recommended health history — published audits have shown many states' screening forms don't even include every AHA-recommended cardiac question. Our intake is built so skipping is impossible: every athlete answers every AHA heart-screening question, dangerous answers automatically stop the process before payment, and a physician reviews the complete record — plus a movement video — before signing. Is an online sports physical legitimate?

Yes. SportSlip is an asynchronous telehealth preparticipation evaluation — a recognized form of medical care — performed under the license of a board-certified physician in the 29 states where he holds one. The clearance is signed with his medical license number and NPI, printed on your school or camp's own form, and every document we issue can be independently verified. It is not a form-signing service: screenings that surface real concerns get a referral and a refund, not a signature.

Will my camp or school accept this?

In almost every case, yes — we complete the actual form they handed you, not a substitute, and staff can verify any clearance in seconds via its QR code. Dr. Kawalek is licensed in 29 U.S. states (see the About page for the full list). If your camp or school has questions or hesitates, contact us and we'll communicate with them directly — verifying the clearance, answering their questions, and correcting the form at no charge if something needs to change.

How can my school, camp, or league verify the clearance is real?

Every clearance PDF carries a QR code. Scanning it (or visiting sportslip.co/verify) shows the clearance's live status straight from our records — valid, expired, or revoked — along with the athlete's sport and the expiry date. A forged or altered PDF fails verification, which is protection most paper physicals simply don't have.

How long does it take?

Standard turnaround is under 24 hours from submission. A same-day rush option is available at checkout.

How much does it cost?

$59 flat for the standard clearance — no insurance, no copay. One clearance is valid for 12 months and covers every sport and every season.

Is an ECG included?

A full ECG isn't part of the standard screen — it requires equipment we can't ship. We apply the AHA 14-element cardiac history, the standard screening recommendation. If anything in the history is flagged, we refer to a cardiologist before clearing.

What if you find something concerning?

If Dr. Kawalek sees anything that warrants follow-up, he'll explain it plainly, give you a written referral to your pediatrician or a cardiologist, and refund the visit. No clearance is signed without confidence.

Is SportSlip HIPAA-compliant?

Yes — a full HIPAA-compliant telehealth framework with professional liability insurance. Your information is encrypted and never sold.

What ages do you clear?

Athletes ages 5 to 18. A parent or legal guardian creates the account and provides the information; the child never uses the platform directly.

What do I need to have ready?

A recent height and weight for your child, a vaccination card (a CDC card, MyChart screenshot, or pediatrician printout all work), your camp or school's specific form if they gave you one, and a phone to record a 30–60 second movement video. A recent blood-pressure reading is optional but helpful.

What is the movement video for?

It lets the physician observe your child's gait, balance, joint mobility, and posture — the functional parts of a physical exam — and screen for Marfan-syndrome features. It's 30–60 seconds, filmed on your phone. Your child's video is reviewed only by Dr. Kawalek and is permanently deleted from our systems 60 days after his decision. We keep his written findings, never the footage.

Which states do you cover?

Dr. Kawalek is licensed in 29 states: AL, AZ, CA, CO, CT, FL, GA, IA, IL, IN, KS, LA, MA, MD, MI, MO, MS, NC, NE, NY, OH, OK, OR, PA, TN, TX, VA, WA, and WI. If your state isn’t listed, we can’t issue a clearance and you won’t be charged.

What is the EKG add-on?

For families who want additional cardiac screening, we offer an optional EKG rhythm review (+$19). If you have an Apple Watch, Kardia, or a 12-lead EKG, you can upload the reading and Dr. Kawalek interprets it and notes it on the clearance. It's entirely optional and never required to be cleared.

Do you offer refunds?

Yes. If your child isn’t cleared (we refer instead), you get a 100% refund. You can also cancel for a full refund any time before physician review begins. And if your state isn’t licensed, you’re never charged in the first place. Once a clearance is issued, the visit is complete and the fee isn’t refundable — but if your camp or school has any questions about accepting it, contact us and we’ll work with them directly to resolve it.
